Transitions in Care
Saint Clare’s offers a four-week program called Transitions in Care, designed to prevent re-hospitalization and improve the continuity of care for persons with chronic conditions across different health care settings and ensure the smooth transition of each change, with no cost to the patient or their insurance provider for any of the resources for the duration of the program.
Visiting Nurse Association (VNA) of Saint Clare's
The VNA of Saint Clare's provides visiting nurses, certified home health aides, physical/occupational/speech therapists, medical social workers, and nutrition counselors to patients in the comfort of their own home.
